Bridging the Chronic Care Gap

We know the problem, chronic disease management accounts for 86% of the United States healthcare costs. Hospitals and healthcare systems struggle with readmissions. Patients feel they don’t have the proper tools to manage care needs effectively and efficiently outside the clinical setting.

Digital technology has improved dramatically in the past several years and there are many options available. But, with all the available products and solutions, why are we not seeing an improvement in chronic care management?

Check out these stats according to a March HealthMine survey of 500 insured consumers:


    • 59% consumers use digital health apps and tools have a chronic condition
    • 52% of the 59% are enrolled in a wellness program
    • 33% of the 59% received health device / app

 

Only 7% of chronic disease patients are using an actual chronic disease management tool.


Why such a small number?


The tools, to this point are useful according to the same poll, but they’re not necessarily being personalized to the individual level. As these digital tools become smarter, and developers and entrepreneurs have more access to data, we will see an increase in usage.


What the adoption of digital health apps means for providers, specifically chronic care:


Providers are not offering patients the resources to manage their chronic diseases, despite 66% of hospitals within the united states currently offering a mobile application. Adoption of RPM applications mean:


  • Lower readmission rates
  • Decrease of costs from elongated hospital stays
  • Increased compliance
  • Patient satisfaction
  • Increase in pharmacological compliance
